On-Line SPC Membership Application
There are three parts to the membership application process:
  1. The first page you will see is the SPC Standard of Conduct page. If you are completing your application strictly on-line, you will be asked to verify that you have read this page and accept the contents. If you are mailing your application, please print this form, sign it and enclose it with your completed form and check.

  2. The application form itself contains information about you and your practice. This is used for possible project referrals by our referral chair person. Please take the time to fill this out accurately. This is explained further on the form.

  3. You can make payment either by mailing a check or by the use of our secure on-line credit card processing service through PayPal. We accept only MasterCard and Visa. Click the PayPal logo for more information.

Web Site Database Participation:
SPC offers members the opportunity to be included in the Web site Membership Database which will offer businesses the ability to directly search for consulting assistance using the SPC Web site.

  • If you choose to be added in the SPC Web site database, answer "YES" to the Web site participation question.
  • If you do not wish to be included in the Web site, select "NO."

Please complete the form even if you do not want to be included in the on-line Web database so that you can participate in the "off-line" referral process.

Having your own personal or business e-mail account and/or Web site are not required for participation on the SPC Web site. Your listing will include your business phone number so that potential clients may reach you by phone.
